Find your document below. Each service shows what it is, what to bring, and what it costs. If yours is not listed, ask.
For every appointment
- The complete document, unsigned. Sign it in front of the notary, not before.
- A current, unexpired government-issued photo ID for every signer: a driver license, state ID card, or passport.
- Every person who is signing. Each signer must be present at the appointment.
- Any witnesses the document requires. Tell us in advance if your document needs them.
The name on the ID should match the name on the document. If a signer does not have a current photo ID, tell us when you request the appointment.
Services
Sworn statements and affidavits
- What it is
- You sign in front of the notary and swear or affirm that what the document says is true. Notaries call this a jurat.
- What to bring
- The statement, unsigned, and your photo ID. You sign it in front of the notary.
- Fee
- $5.00 per notarization, plus the travel fee.
Powers of attorney
- What it is
- The person giving the power tells the notary that the signature is theirs and that they signed willingly. Notaries call this an acknowledgment.
- What to bring
- The complete document, a photo ID for the person giving the power, and any witnesses the document calls for. The person giving the power must be present.
- Fee
- $5.00 per notarization, plus the travel fee.
Deeds and real estate documents
- What it is
- Each signer tells the notary that the signature is theirs and that they signed willingly.
- What to bring
- The complete document and a photo ID for every signer. If more than one person signs, each one must be present.
- Fee
- $5.00 per notarization, plus the travel fee.
Vehicle title forms and bills of sale
- What it is
- A notarized signature on a title transfer or a bill of sale.
- What to bring
- The title or form and a photo ID for each person whose signature is being notarized. Leave the signature lines blank until the appointment.

Consent forms for a minor's travel
- What it is
- A parent or guardian signs a letter or form giving permission for a child to travel, and the notary notarizes that signature.
- What to bring
- The form, unsigned, and a photo ID for each parent or guardian who is signing.
- Fee
- $5.00 per notarization, plus the travel fee.
Business documents
- What it is
- Contracts, vendor and bank forms, and other business paperwork that calls for a notary. We come to your office.
- What to bring
- The document and a photo ID for each person signing, plus anything the document or the office that asked for it says is needed.

- $5.00 per notarization, plus the travel fee.
Oaths and affirmations
- What it is
- A spoken oath or affirmation, given by the notary.
- What to bring
- Your photo ID, and the form the oath is recorded on if there is one.
- Fee
- $5.00 per notarization, plus the travel fee.
Hospital and care facility visits
- What it is
- We come to hospitals, nursing homes, assisted living, and rehabilitation centers to notarize a patient's or resident's signature.
- What to bring
- The document and the signer's current photo ID. Check the facility's visitor rules before you book. The signer must be awake and aware at the appointment and able to tell the notary that they understand they are signing and want to. If the signer cannot do that, the notary cannot go ahead.
- Fee
- $5.00 per notarization, plus the travel fee.
Absentee ballot affidavits
- What it is
- The notary notarizes the affidavit that goes with your Oklahoma absentee ballot.
- What to bring
- Your ballot affidavit, unsigned, and your photo ID.
- Fee
- No charge, and no travel fee when the visit is only for a ballot affidavit.
Loan document signings
- What it is
- Refinances, purchases, home equity loans, and reverse mortgages, for title companies, escrow officers, and signing services.
- What to bring
- Send the order details: the signing location, the date and time window, the type of loan, the number of signers, and how the documents will arrive and be returned.
- Fee
- Quoted per signing.
Not sure what kind of notarization your document needs? The document itself, or the office that asked for it, will say. A notary cannot choose it for you.

The travel fee is separate from the notary fee, and we agree on it with you before the appointment. There is no travel fee when the visit is only for an absentee ballot affidavit.
How the total works
The total is $5.00 for each notarization plus one travel fee for the visit. A visit in Moore with two notarizations is $10.00 in notary fees plus the $25 travel fee: $35.00 in total. We confirm the number of notarizations and the total with you before the appointment.

What a notary does not do
A notary confirms who is signing and witnesses the signature. A notary does not prepare documents, fill in forms, explain what a document means, or give legal advice. If you have questions about what a document says or whether you should sign it, talk with an attorney before the appointment.
Common questions
Can you notarize a document that is already signed?
Bring it unsigned when you can. If it is already signed, tell us when you request the appointment. Some documents can still be notarized, and some must be signed again in front of the notary.
Can you tell me which form I need, or help me fill it out?
No. A notary does not prepare documents, fill in forms, explain what a document means, or give legal advice.
Does everyone have to be there?
Yes. Every person whose signature is being notarized must be present with their ID.
Do you bring witnesses?
No. If your document requires witnesses, arrange them and tell us in advance.
How soon can you come?
Appointments are by request, on weekdays, evenings, and weekends. Same-day appointments are often possible; text (405) 256‑4678 to ask. Requests sent by the form or by email get a reply within one business day.
